Privacy and Consent to Use Personal and Medical Information
Belconnen Specialist Centre respects your privacy and is committed to complying with the privacy law and the code of professional conduct of the profession of all the allied health providers to ensure your privacy is protected at all times.
Belconnen Specialist Centre will seek your consent to collect personal information from you. The main reason we collect information from you is to enable us to provide you with a quality service. The information we collect allows us to properly assess your situation, provide appropriate information and advice, and assist you in achieving your health goals.
Your personal health information and medical record may be collected, used and disclosed for the following reasons:
- For communicating relevant information to other treating doctors, specialists or allied health professionals
- For follow-up appointments/reminder/recall notices
- Accounting, Medicare and Health Insurance procedures
- Quality assurance activities
- For legal disclosure as required by a court of law (e.g., subpoena or court order)
- For research purposes (your information will be de-identified)
- For clients with a DVA referral, please note that the Department of Veterans' Affairs may require a copy of your treatment or meal plan at any time.
A consent form will be provided to you at your first visit with Fisher Allied Health. Please speak to reception if you have any questions or concerns.
